Certificates of Origin

A Certificate of Origin is a document required by foreign governments that validates the country of origin of where the product was manufactured. The certificate of origin must be signed by the exporter, notarized, and then validated by a locally designated agency such as the Manhattan Chamber of Commerce (MCC) in order to be acceptable and valid.

The MCC offers Certificates of Origin for goods made in the United States only. Certificates may be obtained by downloading the form below, by contacting the MCC office at 212-479-7772 or by sending an email to info@manhattancc.org.

  1. The MCC affidavit must be completed with the information requested and notarized. The affidavit releases the Chamber from any liability with regards to the goods. It can be downloaded below, e-mailed or faxed to you.
  2. Bring the signed affidavit to the Chamber office together with the certificate of origin (can also be downloaded below) and the documents regarding the goods (e.g. original invoice). Documents must indicate that the goods are made in the United States.
  3. Please contact the MCC office for an appointment. We will stamp the certificate of origin by appointment only.
  4. There is a fee of $30 for each certificate we stamp for non MCC members. MCC members receive the first 20 certificate stamps free and $10 for each additional certificate. You can pay by cash, credit card or check.
